Meningioma

A rare, mostly benign, primary tumor of the meninges (arachnoid cap cells), usually located in the supratentorial compartment, commonly appearing in the sixth and seventh decade of life, clinically silent in most cases or causing hyperostosis close to the tumor and resulting in focal bulging and localized pain in less than 10% of cases. Additional features may include headache, seizures, gradual personality changes (apathy and dementia), anosmia, impaired vision, exophthalmos, hearing loss, ataxia, dysmetria, hypotonia, nystagmus, and rarely spontaneous bleeding.

Edema

An abnormal accumulation of fluid beneath the skin, or in one or more cavities of the body.

Our imaging studies had as a main hypothesis a frontal Meningioma because of Dural tail sign, lack of edema, contrast enhancement pattern and extra axial location in the supratentorial region, in the frontal lobe, which is uncommon to for a hemangioblastoma.

To our knowledge, this is the first case in which a patient presented with altered mental status from both metabolic (myxedema coma) and structural diseases (frontal meningioma) with vasogenic edema and midline shift.

Sotoudeh H.
Radiol Case Rep. 2018;13(5):1069-1071.
Although ischemia has been previously reported within the peripheral parenchymal edema surrounding a meningioma, in this patient there was no evidence of edema in the right cerebellum on magnetic resonance imaging.
