Meningioma

A rare, mostly benign, primary tumor of the meninges (arachnoid cap cells), usually located in the supratentorial compartment, commonly appearing in the sixth and seventh decade of life, clinically silent in most cases or causing hyperostosis close to the tumor and resulting in focal bulging and localized pain in less than 10% of cases. Additional features may include headache, seizures, gradual personality changes (apathy and dementia), anosmia, impaired vision, exophthalmos, hearing loss, ataxia, dysmetria, hypotonia, nystagmus, and rarely spontaneous bleeding.

Acute postretinal blindness: ophthalmologic, neurologic, and magnetic resonance imaging findings in dogs and cats (seven cases).
Seruca C, Rodenas S, Leiva M, Pena T, Anor S.
Vet Ophthalmol. 2010;13(5):307-14.
Of all lesions detected, five were consistent with intracranial tumors (two meningiomas, one pituitary tumor, two nasal tumors with intracranial extension), and one with bilateral optic neuritis that was confirmed by cerebrospinal fluid analysis.

Presumed bilateral optic nerve sheath meningiomas presenting as optic neuritis.
Sawaya RA, Sidani C, Farah N, Hourani-Risk R.
J Neuroophthalmol. 2008;28(1):55-7.
This is an unusual presentation for optic nerve meningioma but a reminder that this entity may mimic optic neuritis.

MRI of optic nerve enlargement in optic neuritis.
Cornblath WT, Quint DJ.
Neurology. 1997;48(4):821-5.
Both had a clinical course typical for optic neuritis but also had a neuroradiologic finding most commonly seen with optic nerve glioma or meningioma, which initially led to an incorrect diagnosis.

Hemangioblastoma of the optic nerve.
Rubio A, Meyers SP, Powers JM, Nelson CN, de Papp EW.
Hum Pathol. 1994;25(11):1249-51.
Magnetic resonance imaging (MRI) showed a prominently enhancing lesion of the optic nerve, thought preoperatively to represent an optic nerve meningioma or optic neuritis.

Anderson D, Khalil MK.
Can J Ophthalmol. 1981;16(1):10-5.
Three patients had sphenoidal meningiomas with compression of the anterior visual pathways, but the initial diagnoses were acute optic neuritis, chronic optic neuritis and glaucoma.
