POEMS syndrome

POEMS syndrome is a paraneoplastic syndrome characterized by polyradiculoneuropathy (P), organomegaly (O), endocrinopathy (E), clonal plasma cell disorder (M), and skin changes (S). Other features include papilledema, extravascular volume overload, sclerotic bone lesions, thrombocytosis/erythrocytosis, and elevated VEGF levels.

Rheumatoid arthritis

Inflammatory changes in the synovial membranes and articular structures with widespread fibrinoid degeneration of the collagen fibers in mesenchymal tissues, as well as atrophy and rarefaction of bony structures.

We thus concluded that POEMS syndrome, steroid-dependent diabetes mellitus, rheumatoid arthritis, RA, and skin vasculitis in our patient were triggered by plasma cell dyscrasia.

We herein report the atypical case of a 44-year-old white man presenting with glomerulopathy, POEMS syndrome, and erythema elevatum diutinum with a few-year history of non-insulin-dependent diabetes mellitus (NIDDM) and seronegative rheumatoid arthritis (RA) as early manifestations of IgAlambda multiple myeloma.

[Castleman tumor, lichen ruber and pemphigus vulgaris: paraneoplastic association of immunological diseases?].
Plewig G, Jansen T, Jungblut RM, Roher HD.
Hautarzt. 1990;41(12):662-70.
There is a remarkable association of Castleman tumours with skin diseases (lichen planus, pemphigus vulgaris, Kaposi sarcoma), neurological diseases (POEMS syndrome, myasthenia gravis, arteritis temporalis, Guillain-Barre syndrome), and internal diseases (nephrotic syndrome, amyloidosis, plasmacytoma, rheumatoid arthritis, thrombotic thrombocytopenic purpura).
