Primary myelofibrosis

A rare myeloproliferative neoplasm characterized by stem-cell derived clonal over proliferation of mature myeloid lineages, such as erythrocytes, leukocytes, and megakaryocytes, with variable degrees of megakaryocyte atypia, associated with reticulin and/or collagen bone marrow fibrosis, osteosclerosis, ineffective erythropoiesis, angiogenesis, extramedullary hematopoiesis, and abnormal cytokine expression.

OCULAR MANIFESTATIONS OF PRIMARY MYELOFIBROSIS.
Lin AL, Burnham JM, Pang V, Idowu O, Iyer S.
Retin Cases Brief Rep. 2016;10(4):364-7.
Repeat bone marrow aspiration and biopsy confirmed the patient's primary myelofibrosis with no progression to acute leukemia despite his high leukocyte count.

Haematologica. 2000;85(9):973-7.
A bone marrow trephine biopsy (after a "dry tap" external aspiration) was consistent with the diagnosis of overt idiopathic myelofibrosis and only residual foci of CLL cells were present.

We discuss the differential diagnoses of fine-needle aspirations of a localized mass from such patients and review the literature of the association between agnogenic myeloid metaplasia and lymphoma.

A 83-year-old man was diagnosed with primary myelofibrosis based on the presence of leukoerythroblastosis, splenomegaly, chromosome 46 XY, a dry tap bone marrow aspiration and fibrosis on bone marrow biopsy, when he was admitted for herpes zoster in June 1987.
